Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction: Gould Type HE 4 B.eakers (SD-41)
In a June 8,1983 telephone conversation between your Mr. T. Elsasser and our Mr. R. E. Lefebvre, Northeast Nuclear Energy Cornpany (NNECO) reported a potential deficiency in the design and construction of Millstone Unit No. 3 as required by Title 10, Code of Federal Regulations, Part 50, Paragraph 55(e).
The potential significant deficiency involved a Gould Type HE 4100 amp frame-size breaker in 480 V MCC which failed when the "C" pole did not close when the breaker was operated. Three-phrase loads will not operate if one pole of the supply breaker is open.              The breaker was purchased on a Category 1 specification, however, it was not supplying a safety load in this instance.
The exact cause of the failure has not been determined at this time. Discussions with the vendor have been initiated and details will be provided to you as they become available.
