The New York Power Authority (NYPA) submits the following special report to address inoperability of a fire pump for more tnan 7 days.
The event requires that a 30-day special report be submitted in accordance with Technical Specifications 3.12.A.1.c and 6.9.B.2.
Summarv of Event Electric driven fire pump 76P-2 was out of service for approximately.16 days and 5 hours due to extended overhaal time.
Descriotion of the Event and Corrective Action On April-6, 1992 at 0412 hours electric driven fire pump 76P-2 was-declared inoperable in order to perform maintenance.on the electric motor. The diesel engine driven fire pump 76P-1, which is functionally redundant, was operable during this~ period. A third fire pump 76P-4, which is also diesel engine driven and automatically actuates upon decreasing pressure, was available during this period.              No credit is taken for this pump in any analysis and the requirements of Technical Specifications do not apply.                                                                                            i The electric motor was removed and shipped offsite for overhaul on April 17, 1992 to determine the location of (and repair) an oil leak on the lower bearing area.
The vendor disassembled, cleaned and overhauled the motor but was unable to find the cause of the oil leak due to its size.                    During the post maintenance test of the motor the leak was determined to                                j result from a crack in the lower bearing backer area.

_ DOCKET NO. 50-333, ELECTRIC FIRE PUMP                                                  Page 2 of 2 INOPERABILITY Subsequently the vendor ordered repair parts, disassembled, repaired and reassembled the motor. . The repair required welding and machining of the new bearing backer. -The motor was'then tested, found satisfactory and returned to the plant site on April 15, 1992.
f-During the installation.of.the motcr on April 17,--1992 a QC hcid was pleced on the. wiring of the motor space heater.                                            The wiring installation. failed to include flat washers and lockwashers which
              .were required by the. procedure.              The tags were removed from the motor c and - the pump was made available but not operable cui April 17, 1992 00515-while the problem was being resolved. The motor was re-tagged cni April- 21, 1992 02100 to rewire the motor space heater. The motor and pump were released to operations for testing on April 22, 1992. The Operations-surveillance test was performed'and the pump was declared operable on April 22, 1992.at
                -0915.
                .In summary, the pump was out'of service for 16 days 5 Hours and 3 1                minutes due to a difficulty in locating and correcting an oil leakion the lower motor bearing.and a'QC hold with regard to an electrical, wiring. procedure.            During this time the pump was available for 4 days 15 hours and /.5 minutes. No additional problems associated with these conditions are anticipated that                                                                        ,
