The subject Bulletin describes failure of General Electric type HFA relays utilizing either nylon or lexan as the coil spool material.
The following corrective actions have been performed at Shoreham with respect to the problems described in I.E. Bulletin 84-02.
: 1.        Replacement of Nylon Coils with Lexan Coils In response to 1.E. Bulletin 76-02 and GE Service Advice Letter 152.1 a survey was conducted of affected relays which included type HFA relays. Relays found to contain nylon coil spools had these replaced with lexan coils. This was reviewed by the NRC Resident Inspector, and Bulletin 76-02 was closed in NRC Inspection 80-11.
: 2.        Replacement of Lexan Coils with Century Series Tefzel Coils The cracking of lexan coils as described in GE Service Advice Letters 152.2 and 152.2a was reported to the NRC as a report-able deficiency (ref. SNRC-694) and was tracked as Inspection Item 82-00-05.          Per SIL-44 and supplements, corrective actions required the replacement of the HFA relay coil with a GE Century Series tefzel coil, or the replacement of the entire relay with a Century Series relay.

          'In order to ensure that the older and problematic HFA relay coils are not inadvertently used as replacement parts, the nylon /lexan coi's have been-removed from the safety-related equipment-storage inventory.
Inspection item 82-00-05 was reviewed by NRC' Region I per-sonnel and was closed in NRC Inspection Report 50-322/83-16.
  -It should.be noted that the replacement program described above has been extended to include non-safety related type HFA relays.
The portion of the_ program involving non-safety type HFA relays is
  -in progress.
  'The following information pertains to the general concerns of the Bulletin. General Electric type HFA relays are employed in various' safety relatedEsystems, including the Reactor Protection System.
A Deficient Items List, maintained by Operational Quality Assur-ance, has.been updated to require that future _ procurement of these type relays as' spare parts requires certification that the relays be supplied with tefzel coils.
LThe Shoreham surveillance and maintenance program includes
    'information. supplied by the equipment manufacturers and vendors regarding required equipment maintenance actions and their frequency. In addition, as demonstrated earlier in this' letter, LILC0 routinely reviews vendor information such as GE Service Information Letters and~ Service Advice Letters and takes remedial
  -action where_ appropriate.
Furthermore, the Shoreham surveillance and maintenance programs include documented program plans and procedures to assure that the safety-related' equipment.is maintained in a state of readiness and operability _so that'it will perform its intended safety function upon_ demand.
The contents of this letter should satisfy the requirements of the written report sought by item 2b of I.E. Bulletin No. 84-02.
I. E. Bulletin 84-02 page 4 of 6 specifies the actions required of plants under construction. Item 2a requests plans and schedules for~ replacing both normally energized and normally de-energized HFA-relays as specified by this bulletin which are utilized in safety-related systems at Shoreham. Since, as discussed above, theacoil assembly replacement called for by this Bulletin was completed, subsequently inspected, and accepted by NRC Region I personnel, there is no need to. provide Shoreham specific plans and schedules.
