This report is filed in accordance with the requirements of 10 CFR 20.311(h),
concerning investigation of a shipment for which we did not receive notification of receipt within 20 days after transfer.
On August 22, 1985, a shipment of solidified / encapsulated filters was made to the Richland, Washington burial site. The normal notification process for previous similar shipments involved notification from the burial site to Westinghouse Hittman Nuclear (acting as broker / transportation agent), followed by notification from the broker to the Humboldt Bay Power Plant. The usual turnaround time for this process was from 16 to 20 days.
Since formal notification was not received by September 16, 1985, the burial site and the broker were contacted by telephone on September 17, 1985. These calls confirmed that the shipment had been received at the burial site on August 23, 1985. Formal notification was completed on September 17 (by telecopy) from the burial site to the broker to HBPP. The cause of the problem was ascribed to a change of personnel at the burial site and a change of the broker address to which the burial site notification was sent.
At no time was there any reason to believe that the shipment had not been delivered as scheduled.
