: 3. Summary of the report:
In the application for Certificate of Compliance No. 9291, Revision No. 11, for the Liqui-Rad package, the certificate holder requested an increase of the lower limit for the torque values on the outer lid bolts. The U.S. Nuclear Regulatory Commission issued Revision No. 11 on February 13, 2019 (ADAMS Accession No. ML18317A352), which changed the lower limit from 30 [+10 / -10] ft-lbs to 30 [+10 / -0] ft-lbs, as specified in the operating procedures provided with the application.
In response to the revised certificate Westinghouse changed its internal procedure for operating the package. However, the internal Westinghouse procedure that was issued on July 25, 2019, listed the torque for the outer lid nuts as 30 [+10 / -10] ft-lb, which did not comply with the operating procedure referenced in the certificate. When Westinghouse noticed the discrepancy on December 18, 2020, it placed a hold on all shipments for this package, until the procedure was revised. Westinghouse stated that the incorrect lower limit on the torque value was incorrectly recorded in the procedure due to human error.
Westinghouse stated that approximately 150 shipments, each containing nine packages, were made from their plant in Columbia SC between July 25, 2019 and December 18, 2020. However, Westinghouse confirmed that there were no known shipments that were out of compliance with the revised certificate of compliance. In addition, Westinghouse confirmed there were no known issues resulting from the lower outer lid lower torque value in their procedures.
