EVENT DESCRIPTION AND PROBALBE CONSEOUENCES E                                          During normal operation at 0632, #21 Emergency Diesel Generator (EDG) tripped on loss t
                                          'of field fifty-five minutes af ter being started for an outage on service transformer
;                                          P-13000-2 and was declared inoperable (T.S. 3.8.1.1.b). #12 EDG was then started and
  <                                        p.aralleled with offsite power but it tripped on loss of field at 0705 due to fluctuations in        -
500 KV system line voltage (T.S. 3.0.3). #12 EDG and #21 EDG were successfully restarted and paralleled at 0735 and 0800 respectively. #21 EDG again tripped at 1445 i                                        on loss of field during load reduction after a successful one-hour surveillance test run. It
                  ,                        was started and declared operable at 1515. One offsite power source remained operable
                                        . during the event. There are no similar events.
CAUSE DESCRIPTION AND CORRECTIVE ACTIONS; EDG #21 tripped due to failure of operating personnel to detect voltage regulator drif t .
and adjust voltage prior to EDG trip upon protective relays sensing a reverse power
;                                          condition.
Contributing to the two trips on June 2,1982, is the condition of #21 voltage regulation
                        <.                which has normally exhibited greater tendency to drif t than has EDG #11 or #12 when
                          -               operating in parallel with a source supplied through the off-site distribution grid. When acting as the single supply to a vital bus, each EDG exhibits adequate regulating
  ;                    :                  capability.
                            $              Background i                          , 1.              EDG #21'has exhibited reactive load drif t for some time. Maintenance records
            / ;                                            document attempts to improve its regulation when in parallel operation since late Q;                                  .<                    in 1980. These include:

: 1.        Governor replacement with an overhauled unit,
: 2.        Load regulator replacement,
: 3.        Voltage regulator replacement,
: 4.        Continuity and condition checks in control circuits, and                  ,
: 5.        On-site consultation with the vendor's technical representative.
None of these activities have disclosed a correctable cause of voltage regulator drift.

: 2. Because the grid interconnection from the plant utili7.es automatic voltage adjusting taps, perturbations in grid voltage may requ re unit generator adjustments on a periodic basis. These sources of voltage change have serious effect on EDG reactive load when EDG's are paralleled to an off-site source.
: 3. It is noteworthy that following the events of June 2,1982, EDG #21 was successfully operated f or one hour while paralleled and loaded to 2500 KW on thirteen occasions during the succeeding nine days. These operations were periormed ior surveillance purposes.
Preventive Action As reported in LER 50-317/82-27/3L a caution label will be affixed to the Control Room Control Panel alerting operators to monitor EDG reactive load when adjusting system line voltage. This permanent label will also serve to remind operators of the necessity to monitor reactive load during EDG parallel operation.
The voltage regulator assembly for EDG #21 will be replaced as an attec,pt to improve regulation. Replacement will be accomplished when EDG #21 can be removed from service upon completion of maintenance which is currently being performed on other emergency power source equipment.
