in accordance with Appendix A Technical Specifications 4.0.3, 4.6.1.3 and 6.9.1.12.b to Operating License NPF-10 for San Onof ra Unit 2, this submittal provides the required 14 day written follow up and Licensee Event Report for an incident involving a missed surveillance on the l
Containment Personnel Air Lock. Prompt notification of the event was confirmed by the facsimile transmission referenced above.
On June 4, 1982, Unit 2 entered Mode 4 at 1700 hours.                  The Containment Personnel Air Lock was subsequently opened for multiple entries by Operations and Maintenance Personnel. Technical Specification 4.6.1.3.a requires that the Air Lock be demonstrated operable within 72 hours following each closing, except when the Air Lock is being used for
,                        multiple entries and then at least once per 72 hours. A maximum allow-
!                          able extension not to exceed 25T of the survell!ance interval (18 hours l                          In this case) is permitted per Technical Specification 4.0.2.a.                  This demonstration of operability, therefore, was required to be completed by 1100 hours on June 8, 1982.
l Due to miscalculation of the allowable time interval by Operations personnel, the test was nor performed until 0915 hours on June 9, 1982.
Test results confirmed the Air Lock operability.
Corrective measures taken include verbal instructions to Operating personnel on the need to follow surveillance requirements stringently, l
ar.d revising the practice of identifying surveillances on the schedule sheets to include the identi f ication of "NO LATER THAN" time and date.
