                                              ,. .    ,e.                P ane . .- .        N.,.,,,-    ..., n ei On June 30,1990 at 1:50 a.m., EDT, the requirements of Seabrook Station Technical Specification 6.11, "High Radiation Area", were not met.
Seabrook Station Technical Specification 6.11, "High Radiation Area", requires individuals 'who enter locked high radiation areas to be qualified in radiation protection procedures and have in their possession a dose rate radiation monitoring device, when not escorted by a qualified Health Physics Technician.
Contrary to this requirement, an auxiliary operator qualified in radiation protection procedures entered a locked high radiation area without possessing a dose rate radiation monitoring desice.
The root cause of this event was personnel error involving a lack of attention to detail.
There were no safety consequences as a result of this event.
The auxiliary operator was counselled regarding the consequences of the event and the need for increased attention to detail. A meeting was held between the Operations Manager and the Health Physics Department Supervisor to discuss this event and actions to prevent recurrence. These actions include the placement of signs containing access requirements at key card readers and keyed doors to locked high radiation areas, access authorization to high locked radiation areas was removed for Operations personnel, and a training Development Recommendation was generated to include a discussion of this event.

On June 30,1990 at 1:50 a.m., EDT, the requirements of Seabrook Station Technical Specification 6.11,                            !
                  'High Radiation Area", were not met.
Seabrook Station Technical Specification 6.11 requires individuals who enter high locked radiation areas to be qualified in radiation protection procedures, when not escorted by a qualified Health Physics Technician, and to have in their possession a dose rate radiation monitoring device. Contrary to this requirement, an auxiliary operator qualified in radiation protection procedures, entered a locked high radiation area for approximately four minutes without possessing a dose rate radiation monitoring device.                      l t
Root Cause The root cause of this event has been determined to be personnel error involving a lack of attention to detail. The auxiliary operator involved in the event was performing routine inspections, and did not observe the 'High Radiation Area, Survey Meter Required' sign posted on the door prior to entry.
Safety Conscauences There were no adverse safety consequences as a result of this event. Health Physics immediately                                  i performed a survey to determine the dose rates in the area. that the auxiliary operator had passed through. Although a small section of this area was greater than 1,000 millirem per hour, the area'that the operator passed through had a dose rate of 0.5 - 5.0 millirem per hour.
Corrective Actions The auxiliary operator involved in this ovent has been counselled 'regarding the consequences of the event and the need for increased attention to detail. Additionally, a meeting was held between the Operations Manager and the Health Physics Department Supervisor to discuss this event and actions to prevent recurrence. These actions include:
: 1. Signs containing access requirements were placed at key card readers and keyed doors that are used for access to locked high radiation areas.
: 2. The access authorization to locked high radiation areas with key card access was removed for Operations personnel. Operations personnel will be required to contact Health Physics for access to locked high radiation areas for routine operations. However, Operations personnel have an override key to gain immediate access in the event of an emergency.
: 3. A Training Development Recommendation was generated to include a discussion of the consequences of entering high radiation areas without observing the established controls for personnel who have access authorization to locked high radiation areas.

This is the first event of this type at Seabrook Station.
