10 CFR 71.95 REPORT EVALUATION FORM Docket No.: 71-9291 Package Model No.: Liqui-Rad Report Submitted By: R. J. Freudenberger, Director, Safety and Safeguards, Nuclear Fuel Services, Inc.; Point-of-Contact: Brad McKeehan, NFS Transportation and Waste Shipping Unit Manager Report Date: November 30, 2017.

Report ADAMS Accession No.: ML17346A686 Review the incoming report to detennine if additional Commission or staff action is warranted. The review should consider whether the report identifies a generic defect or problem with the package design and the safety significance of the issue.

Note that a high safety significance represents a potential for significant radiation exposure, medium safety significance represents a potential for some moderate radiation exposure, and low safety significance represents litUe or no potential for radiation exposure.

1. The report identifies:

_ Significant reduction in the effectiveness of a package during use;

_ Defect with a safety significance;

_:!:... Shipment in which conditions of the approval were not observed .

2. What is the safety significance? _ High Medium ./ Low
3. Summary of the report:

On October 10, 2017, Nuclear Fuel Services, Inc. {NFS) performed a shipment of Liqui-Rad Transport Unit Packages {LR-230s) [certificate of compliance number {CoC No.) 9291, Revision 9, package identification No. USA/9291/B{U)F-96] to the Westinghouse Columbia Fuel Fabrication Facility {WFFF or Westinghouse). The shipment included nine (9) Liqui-Rad packages with uranyl nitrate solution.

On October 11, 2017, during unloading operations of the LR-230s at the WFFF site, WFFF notified WesDyne International {who notified NFS), that one { 1) of the eleven { 11) secondary lid bolts of package number LR-004 was not tighten as specified in the Safety Analysis Report for Packaging for the Liqui-Rad Transport Unit {thereafter, Liqui-Rad SAR). Package number LR-004 contained 865.4 liters of 4.95% of uranium-235 (235U) low enriched uranyl nitrate solution. The following sections of the SAR are relevant to this incident:

a. Section 2.4.3 of the SAR provides a description of the bolts used for closing the primary and secondary lid of the package.
b. Section 2.10.9 of the SAR describes the LR-230 package as leak tight.
c. Section 7.1.2 {c) of the Liqui-Rad SAR specifies using 75 ft-lb [+1 O -0] of torque to tighten the secondary lid bolts.

10 CFR 71.95 REPORT EVALUATION FORM Docket No.: 71-9291 Package Model No.: Liqui-Rad Report Submitted By: R. J. Freudenberger, Director, Safety and Safeguards, Nuclear Fuel Services, Inc.; Point-of-Contact: Brad McKeehan, NFS Transportation and Waste Shipping Unit Manager Report Date: November 30, 2017.

Report ADAMS Accession No.: ML17346A686 Review the incoming report to determine if additional Commission or staff action is wananted. The review should consider whether the report identifies a generic defect or problem with the package design and the safety significance of the issue.

Note that a high safety significance represents a potential for significant radiation exposure, medium safety significance represents a potential for some moderate radiation exposure, and low safety significance represents little or no potential for radiation exposure.

3. Summary of the report (Continue):

Prior to transport, the secondary lid of each LR-230 is leak tested to a sensitivity of 1 x 10-3 ref-cm 3/sec per ANSI N14.5 under normal conditions of transport. The LR-230 LR-004 successfully passed the required leak test. In addition, the report dated November 30, 2017, submitted by NFS, states the following in the attachment, item No. 3, to its report:

"Downblending Operations performed a hypothetical leak test on a separate LR-230 package with a closure bolt missing on the secondary lid and the other eleven ( 11) bolts torqued to 80 ft-lb and the leak test was successfully passed. Therefore, based on the successful results of the leak test, under Normal Conditions of Transport, the remaining eleven ( 11) bolts could have performed the same function as the components and systems that failed [one (1) bolt not torqued appropriately, as required] during the event.

The report submitted by NFS on November 30, 2017 states that there were no issues with the following:

a. any of the other eight (8) LR-230 packages,
b. the sixteen (16} closure lid bolts on the primary lid no similar previously, and
c. other LR-230 packages (i.e., NFS did not identify similar issues with other LR-230 packages).

The probable cause of not tightening the bolt was human error.

4. Corrective actions taken by the licensee:

Westinghouse unloaded the contents of LR-004 without incident. In addition, per letter dated November 30, 2017, NFS took the following actions:

a. documented the event in NFS' Problem Identification, Resolution, and Correction System (PIRCS} as Problem 60977,

Report ADAMS Accession No.: ML17346A686 Review the incoming report to determine if additional Commission or staff action is warranted.

4. Corrective actions taken by the licensee (Continue):
b. conducted an investigation of the event that included the following:
i. briefed Downblending Operations personnel about the event, ii. discussed the procedure for tightening secondary lid bolts with the Downblending Operators, iii. interviewed the Downblending Operators about unusual events related to LR-004, and iv. checked and verified installation and torque for the secondary lid in LR-230 Trailer Fill Runsheet (RS-409-458-440) for LR-004.
c. revised standard operating procedure 409, Section 6, to add a peer check and document it when tightening the bolts of the secondary and outer lid to verify compliance with the torque requirements.
5. Staff comments:

The NFS' document identification number related to this event is 38G-17-0028. On April 2, 2018, NFS provided information to support the review of the event reported by letter dated November 30, 2017.

6. Staff conclusion:

~ The report does NOT identify generic design or license/certificate issues that warrant additional Commission or staff action. This report is considered closed.

_ There is a need to take additional action. Provide a summary of the bases and recommended actions:
