==Dear Mr. Johnson:==

On September 4,1986, we verbally notified your Mr. I. Barnes of a deficiency involving a broken cotter pin on a snubber. Our evaluation of this deficiency and other issues have provided the basis for expansion of this issue to encompass an increased scope of pipe support construction deficiencies. We have submitted an interim report logged TXX-6012, dated October 3, 1986. We are reporting this issue under the provisions of 10CFR50.55(e) and the required information follows.
DESCRIPTION Based upon an evaluation of several review programs, construction deficiencies have been identified involving completed pipe support installations. In order to assess the full extent of these conditions, an extensive re-inspection program is required. The magnitude of this inspection program, and the evaluation required to assess the effects of the finding on the safety of plant operations, are significant.
The scope of this issue includes all Unit 1 and Common safety class piping and supports.
Review programs involved in the identification of the construction deficiencies and the subsequent need for reinspection include: several Corrective Action Request (CARS) stemming from inspection activities related to the Piping and Support Construction Adequacy Program; an assessment by Stone & Webster Engineering Corporation of the CPSES program established in response to NRC IE Bulletin 79-14; and TUGC0 actions required to augment the piping stress re-qualification program.
SAFETY IMPLICATIONS In the event these construction deficiencies had remained undetected, the integrity of the completed pipe support installations and piping systems could not be assured under normal operating and accident conditions.

TXX-6027 October 16, 1986 Page 2 of 2 CORRECTIVE ACTI0tl A reinspection program, called the Hardware Validation Program (HVP), will be conducted to re-verify the adequacy of pipe support installations. The HVP will include construction and engineering interfaces to the extent modifications and rework requirements are identified, implemented, and inspected. Completion of the HVP will result in documented pipe support installations which meet the established design and regulatory criteria.
The HVP is currently scheduled to begin January 15, 1987. Reports will be submitted on a quarterly basis to provide the status of the results of the HVP. We anticipate submitting our next report on this issue no later than February 2, 1987.
