NPDES Permit No. LA0042731 River Bend Station - Unit 1 Gulf States Utilities Company (GSU) regrets the discrepancy noted in your letter of August 25, 1989. There was indeed a violation of our federal permit effluent limitation for Total Suspended Solids (TSS) in Outfall 005 on June 28, 1989. Contrary to the statement on the Discharge Monitoring Report (DMR)   for June, this excursion was not described in our {{letter dated|date=July 3, 1989|text=July 3,1989 letter}}.
The latter was a follow-up to a telephone notification of bypasses at Outfalls 004.and 005.
On June 28 1989, a :evere storm w ent caused an estimated 30,000 gallons of.
partially-treated sanitary wastewater to overflow from              the  0.25-acre facultative/ anaerobic polishing pond which normally produces . effluent to Outfall_005. The estimate is based on a bypassing flow which appeared at worst to approach 100 gallons per minute and persisted for five hours (from about 8:00 a.m. to about 1:00 p.m.). Since flows during the last hour or so were somewhat reduced, the total volume bypasses was probably somewhat less than the above estimate. A grab sample of this bypassed wastewater, collected at 11:25 a.m., contained 111.2 mg/l of TSS in violation of of the maximum limit of 45 mg/1. The noncomplying concentration was recorded as 112 mg/l on the June, 1989 DMR for Outfall 005. When combined with the other two grab sample results for June, the instantaneous concentration of the bypassed water resulted in a violation of the daily average limitation for TSS, reported as 82 mg/l in the DMR and tallied as the second excursion for the month.
An original draft of the follow-up letter to your agency addressed the daily maximum TSS violation just described.       Concurrently, a similar follow-up letter was drafted for the Louisiana Department of Environmental Quality (LA-DEQ). The state permit (No. WP0409) daily maximum effluent limitation for TSS at Outfall 005 is 135 mg/l and therefore the relevant passage of the draft version to LA-DEQ stated, " Overflow from the pond met TSS and pH limits Unfortunately, when the final versions of the respective letters were word-processed, both contained the statement about Outfall 005 intended only for the LA-DEO. With the texts of both letters nearly identical,               the discrepancy was overlooked during final review prior to signature 890908o037 890831                                                I f
