AUGHENTED INSPECTION TEAN DISPATCHED (AIT); STEAM GENERATOR TUBELEAK(UPDATE)
The AIT completed its inspection at 8:00 a.m. MST, March 25, 1993, with an exit meeting at the Palo Verde site.
The team's major findings include:       (1 the isolaa. ion of the steam gen &rator tube rupture (SGTR) was delay)ed by the deliberate use of the
    ,5      gency Operating Procedures (COPS) and a shift tu         ver; 2) the Palo 1 event classification procedures for a SGTR resu in higher
         ..gency classification than NRC guidance suggests; ( ) t operators did not recognize that leakage from the reactor coolant;systy was in excess of charging pump capacity for about six and one-lhalf finutes prior to the manual reactor trip, as a result, the operators Eclass@ted the event as an ALERT rather than a SITE AREA EMERGENCY as fequired by their procedures (NRC guidelines would classify the event as in All2T); (4) the E0Psdidnotappeartoprovideanoptimumresponsetothifulbrangeof SGTR events in a timely manner; and (5) site assembly and accountability tas completed after six hours and forty-four minutes and not within                             f, thirty minutes as required by Palo Verde procedures.                                          U      p The cause of the tube leak has not 'been determined. Unit 2 is currently in a planned refueling outage during which time the steam generator tubes will be inspected.
