AUXILIARY FEED SYSTEM VIBRATION PROBLEM (PIPE FAILURE)
Significant system vibration was observed in two separate locations on the auxiliary feedwater (AFW) system while feeding steam generators at low flow rates.
After approximately 5 minutes of vibration a 1-inch drain pipe failed. The system was shutdown and declared inoperable. This failure occurred early Sunday morning November 22, 1987. The AFW system had been declared operable early Saturday morning after repairs and testing had been completed following earlier vibration induced pipe and pipe support failures.
At the time of the event, STP Unit 1 was heating up to hot standby (mode 3) in preparation for initial criticality. Temperature has now been reduced to hot shutdown (mode 4) conditions.
This occurrence will impact the South Texas Project Unit I startup schedule.
The licensee is investigating the failure and the NRC is monitoring their activities.
Neither the licensee nor the NRC plans to issue a press release at this time.
The state of Texas will be informed.
Region IV received notification of this occurrence by telephone from the senior resident inspector at the South Texas Project on November 22, 1987. Region IV has informed ED0/NRR/PA.
