FAILURE TO REPORT AND DOC 1' MENT CONTROL ROD MOVEMENT 7RROR (UPDATE)
As of December 3, 1992, the Office of Investigations, assisted by.
a team of Region III inspectors, is continuing its onsite investigation of a September 18, 1992, incident in which c control rod movement error occurred and was not documented or reported to licensee management. The incident was reported by the licensee on' November 24, 1992, after management became aware of it.
The licencee completed its investigati.n on December 2.        Based on its investigation, the licensee terminated the employment of four of the five individuals involved in the incident. The fifth employee was suspended for a week without pay.
The licensee concluded that severa] inappropriate actions were part of the incident:     (1) inadequate self check-and second verification of the movement of the control rod;        (2) inadequate following of procedures in correcting the rod movement error;        (3) preparing documentation which incorrectly recorded the control rod movement error; and (4) decisions by the individuals not to report, log, or discuss the incident witl. anyone else.
The licensee's conclusions were provided in a report to all employees issued on December 2, 1992, by the plant manager.        The plant manager also has met witn cach operating shift to discuss the incident. The manager emphasized that the individuals were terminated for a " demonstrated lack of trustworthiness and irtegrity" and not for a " simple mistake." The findings of the NRC investigation are under review for possible further action.
Region III will continue its augmented shift coverage at the plant-with inspectors present during each 8-hour shift for several more days.
The State of Illinois will be informed of this updated information. The information in this preliminary notification has been reviewed with licensee management.
