DISCOVERY OF RAD 10GR/ PHY DEVICE BY 11 EMBER OF PUBLIC On March 4, 1991, the Hammond, Indiana, Fire Department notified the NRC Operations Center that a radiography device had been found on a street by a member of the public who brought the device to the fire department. The fire department surseyed the device and notified both the tiRC and the licensee (the device had a tag identifying the licensee).
Region 111 (Chicago) instructed the fire department on appropriate precautions and dispatched two inspectors to Hammond. The inspectors determined that the device's sealed radiation source was in its shielded and locked position.                          The device contains a nominal 62-curie iridium-192 source. The device was then turned over to the licensee.
Radiation levels were 70 millirern per hour at contact and 2.5 millirem per hour at 1 meter. The NRC inspectors estimated the individual who discovered the device received a radiation dose of less than 5 millirems, s                       m licensee and NRC Region 111 are investigating the circumstances surrounding the                  g scovery of the dev ce. At the time it was found by the private citizen, licensee i
management was not aware of its loss. The incident will be reviewed in a future NRC inspection.
The State of Indiana was notified.                    The information in this Preliminary flotification has been reviewed with licensee management.
Region 111 was notified of the device's discovery at 5:15 p.m. (CST) on March 4, 1991.           This information is current as of 10 a.m., March 5, 1991.
