Pt.lisades Covert, MI 49043                                   Notification of an Unusual Ever.t X Alert Site Area Emergency Docket No. 50-255                                  General Emergency Not Applicable Suoject: LOSS OF SHUTOOWN COOLING At6:37p.m.(CDT), October 15, 1987 with the unit in cold shutdown for maintenance, operators tripped the pressure       operating within           shutdown coolant pump in response to alarms and indications of fluctuating the pump.
The cperators then noticed that one of the four Loop isolation valves for shutdown cooling was cycling open and closed. This rapid cycling was determined to be the cause of the pressure and flow variations within the shutdown cooling pump. Operators resolved the problen by closing the valve and shutting off power to prevent it from reopening.
Twenty six minutes later, the shutdown cooling pump was restarted. During the time shutdown cooling was shut off, about 1,000 gallons of primary coolant had drained from the system. The water temperature also increased 37 degrees, to 129 degraes F. With the pump restarted, the primary coolant system was refilled to the previous level.
The cause of the valve cycling was determined to be incorrectly placed jumpers. These jumpers re being used by a testing company (Movats) to individually test the valves. The licensee aid that an electrical drawing was read incorrectly and that a jumper was placed on the wrong terminal.      Na verification or second level review of the tenninal selection by the Movats electrical engineer was perfonned.
All Movats work was suspended and a review of the work performed. The incorrectly placed
  . jumper was removed at 7:05 p.m. (CDT). All remaining Movats jumpers were then removed. The licensee is expected to revise its procedures for controlling jumpers under a work order before resuming work.
The licensee der.lared and terminated an " alert" at 7:49 p.m. (CDT) under its emergency classi-fication system because of the loss of shutdown cooling.
The Resident Inspector responded to the event and is reviewing the licensee's course of action.
The State of Michigan will be notified.
