CONTROL ROD MANIPULATION ERRORS In two instances, at 3:01 a.m. (EST) and again at 5:06 a.m. (EST) on October 31,1986, while conducting a reactor plant startup and at 17 percent pouer, a reactor operator moved a control rod cut of its proper sequence.                               '
In the first event the operator inadvertently skipped a step in his rod withdrawal procedure and pulled the next rod in the sequence (in the same group) out one notch (6 inches). The cp rator immediately recognized his error and notified his supervisors. Licensee management and ai. onsite NRC Restart Team inspector were also notified. With the concurrence of his sup;rvisors and Reactor Engineering, the rod was reinserted to its proper position and the ccrrect rod sequence was restored. A Reactor Engineering evaluation indicated that there was little technical or sMety significance to the error.
In th] second event, the same operator had completed pulling each of the four rods in the saketed group one notch. At this point an indication problem arose and the operator stopp;d rod pulls to resolve it. When he started to pull rods again, he failed to select th; n:xt rod in the sequence.      Instead, he pulled the last rcd that had been mcVed, one additional notch. The operator, once again, immediately recognized his error and notified his supervisors. Licensee management and the on-shift NRC inspector were again informed.
The shift supervisor ordered all rod movements stopped and the operator was replaced. With the concurrence of Reactor Engineering, after a technical and safety evaluation, the rod was reinserted to its proper position.

OCCURRENCE--PNO-III-86-125 Date Octob:r 31, 1986 PJELI While there is little technical or safety significance to these errors, the failure of management procrams to prevent the recurrence of rod manipulation errors is of concern to Region III (Chicago). Resident Irspectors and regional management are closely following and evaluating the licensee's corrective actions. The licensee, after consulting with Region III management, has implemented a double verification for all rod movements effective this date.
The State of Michigan will be notified.
The ensite NRC Restart Team inspector was notified of the first incident at about 3:30 a.m.
