1MSIV FAILURES TO CLOSE DURING SURVEILLANCE
           ..On February 6,1986, _during routine. closure surveillance of the MSIV's, several hours
           = following:a reactor scram (at 9:55 p.in. on February 5) one (inboard) MSIV failed to
           ~close.and.Its redundant-(outboard) MSIV_was shut only.after several attempts by the
      '       optrators to do. so~. .The main steam lines.are currently isolated, although one inboard
:MSIV remains'open. . Containment is being de-inerted to allow entry to investigate and-repair the:open valve. Investigation of the outboard valve-1s also under way. The r2sidentl inspector and Projects Section Chief are onsite'following licensee actions.
