Box 236, Hancocks Bridge, New Jersey 08038-0236 AUG 1 6 2005                 NucluarlC LR-N05-0421 United States Nuclear Regulatory Commission Document Control Desk Washington, DC 20555 SPECIAL REPORT 354/05-006-00 HOPE CREEK GENERATING STATION FACILITY OPERATING LICENSE NPF-57 DOCKET NO. 50-354 This special report is being submitted pursuant to the requirements of Hope Creek Technical Specification (TS) 3.3.7.5, "Accident Monitoring Instrumentation" Action 81, due to the South Plant Vent (SPV) radiation monitoring monitor system (RMS) being inoperable for greater than 72 hours.
On August 2, 2005 at 0918, TS 3.3.7.5 was entered due to the SPV RMS being inoperable for a scheduled channel calibration. TS 3.3.7.5, Action 81 requires that "With the number of OPERABLE accident monitoring instrumentation channels less than required by the Minimum Channels OPERABLE requirement, either restore the inoperable channel(s) to OPERABLE status within 72 hours, or: (a) Initiate the preplanned alternate method of monitoring the appropriate parameter(s), and (b)
Prepare and submit a Special Report to the Commission pursuant to Specification 6.9.2 within 14 days following the event outlining the action taken, the cause of the inoperability and the plans and schedule for restoring the system to OPERABLE status."
The preplanned alternate method of sampling was implemented when the channel was removed from service for the calibration and remained in effect following the 72 hours allowable outage time (AOT).
The AOT was exceeded due to incorrect calibration procedure instructions and damage to instrument circuit boards while performing trouble shooting activities. The circuit boards were replaced and tested satisfactory. The calibration procedure was revised and approved. The combination of these two problems delayed the return of the channel to OPERABLE status within the AOT. Corrective action included coaching personnel involved with trouble shooting activities and determination as to the cause of the incorrect calibration procedure. The procedure instructions were incorrect as a result of failing to revise the procedure following a RMS detector replacement. The monitor was declared OPERABLE following the procedure revision and completion of the calibration. The channel was returned to service on August 6, 2005 at 1906 hours.
