Haddam Neck Plant Request for Temporary Exemption from Single Failure Requirements By {{letter dated|date=April 22, 1986|text=letter dated April 22,1986}},(I) Connecticut Yankee Atomic Power Company (CYAPCO) requested a temporary exemption from the single failure requirements applicable to small break Loss of Coolant Accidents (LOCA) at the Haddam Neck Plant. This exemption request was the subject of a meeting between the NRC and CYAPCO on April 23, 1986. At the conclusion of the meeting, the Staff indicated that, based upon the technical merits of the proposal, it intends to grant the requested exemption in time to support start-up from the current outage, provided CYAPCO formalized several commitments made durir:g the course of the meeting. The purpose of this letter is to fulfill the commitment made during the April 23 meeting to formalize the agreements described below.
The first issue concerns Emergency Operating Procedure (EOP) 3.1-4, Loss of Coolant. EOP 3.1-4 is the procedure which describes operator actions to be taken to assure the availability of high pressure recirculation. (Revision 29 of this procedure was provided as Attachment A to the April 22, 1936 le tter.)
During the meeting, CYAPCO explained that a number of refinements and enhancements to this procedure were planned to be completed prior to start-up from the current outage. Two specific changes were discussed in some detail during the April 23 meeting. The first issue concerns MOV-24 and MOV-374, which are the valves for which the exemption request is being sought. These non-single failure proof valves are required to change position to align High Pressure Safety injection (HPSI) in the recirculation mode. The preferred method for accomplishing this realignment 's through remote operation. During the meeting it was described that if remote operation is not successful, the valves could be repositioned manually. EOP 3.1-4 will be revised to describe this option specifically.

The second aspect of EOP 3.1-4 concerns the use of inventory remaining in the Refueling Water Storage Tank (RWST) after the initial 100,000 gallons of RWST inventory are injected in the short term following a LOCA. It was noted that if the operators are unsuccessful in aligning ar.y method of high pressure recirculation, that additional inventory remaining in the RWST could be used to either forestall or eliminate the need for high pressure recirculation. The procedure is being revised to articulate this option. The above two and other appropriate procedural enhancements will be implemented prior to reactor criticality.
The second commitment made during the meeting again relates to MOV-24 and MOV-874. As described in the April 22 letter and discussed during the meetin these two MOVs are currently required to be locked in the normal position with breakers removed by Technical Specification requirements. To provide increased assurance of the availability of HPSI recirculation, CYAPCO plans to implement monthly surveillance tests for these two MOVs and to docket a license amendment request shortly to authorize such surveillance. This letter will confirm that independent verification will be an element of this amendment request.      Specifically, the conduct of the monthly surveillance for valve operability, and assurance that the valves are returned to their proper position following surveillance, will be independently verified.          This independent verification provides increased assurance of valve operability and proper alignmen t.
The third issue discussed during the meeting concerns our plans for conducting operator training on the revisions to EOP 3.1-4. Because of the unique aspects of this procedure, special training will be conducted to assure thorough operator awareness of both the actions to be taken and the reasons behind those actions.
The training will be completed before any individual operating shif t goes on duty once the reactor is critical.
With the docketing of this letter, it is our understanding that the Staff has a!!
the necessary information required of CYAPCO to grant the subject exemption.
We will continue to keep you advised verbally regarding the schedule f er reactor criticality and subsequent operation.
