AECM-82/268, 6/15/82 AECM-82/189, 4/26/82 On March 26, 1982, Mississippi Power & Light Company notified Mr. R.
Butcher, of your of fice, of a Potentially Reportable Deficiency (PRD) at the Grand Gulf Nuclear Station (GGNS) construction site. The deficiency concerns the failure of a switching relay in the Unit 1 Division I Standby Diesel Generator.
As previously reported, this deficiency has been determined to be reportable under the provisions of 10CFR50.55(e) and 10CFR21 for Unit 1 and 10CFR50.55(e) for Unit 2. Information obtained as of this date is included in our attached Final Report.

A. Descrigtton of the Deficiency.
During testing, the Unit 1 Division I Standby Diesel Generator started in                                                         i response to the simulated LOCA signal and reached rated ' speed but failed to reach rated voltage. It was found that the relay which transfers voltage control- from the manual voltage regulator to the automatic volt-                                                         <
age regulator was inoperative. As a result, the voltage output of the                                                            i generator did not come up to rated voltage af ter the diesel' had started and come up' to rated speed.                                                                                                      j t
j                B. Analysis of Safet,y Implications 1
In the event that testing had not detected this-failure, and.the Division I diesel had remained in manual control and had been required to start                                                           (
and carry all of its normal loadu, the voltage could have been so low
;                      that the diesel output breaker would not have closed due to a low output                                                         !
:                      voltage interlock. This would result in the loss of two ECCS pumps, the                                                          ;
!                      RHR "A" pump and the LPCS pump.

: 5. The date on which the information of such deficiency ... was obtained.
Mississippi Power and Light received information of the deficiency on March 25, 1982. We reported the deficiency to Mr. Ross Butcher, of your office, as a Potentially Reportable Deficiency on March 26, 1982. Since that date MP&L has filed two (2) Interim Reports to inform the Commission of the progress and status of the evaluation of this deficiency. An evaluation for Part 21 has been completed and the MP&L " Responsible Of ficer," Mr. J. P. .McGaughy, Jr., has been notified.

A. Corrective Actions Taken f
The failed relay on the Unit 1 Division I diesel was replaced with 4
another relay of the same design, which tested satisfactorily. The cir-cultry of the relays for both the Unit I and Unit 2 diesels will be modified, per DCP-82/3538, prior to 5% power testing for Unit 1, to           <
remove voltage from the relays once the relays are latched in either an open or closed position. A temporary alteration has been installed on Unit I which will physically retain voltage control in the " Automatic" mode until the Design Change is incorporated.
Our Architect / Engineer and their vendor have investigated the cause of    t the design error, and have determined that the ori6 inal design intent was ,
to have the circuit operate exactly as it presently operates, with constant voltage applied to the relay. In 1974-75 when this circuit was designed, the relay, Magnecraf t W88LCPX-12, was advertised by the          !
manufacturer, as being a continuous duty relay. However, some time later, at least by 1981, the Magnecraf t catalog no longer advertised the j                   relay as having continuous duty capability. Therefore, the original        i design was correct, but due to a nanufacturing specification change, the    ,
relays installed were pulse-type relays only.
