AECM-82/251 On May 4, 1982, Mississippi Power & Light Company notified Mr. F. J.
Long, of your office, of a Potentially Reportable Deficiency (PRD) at the Grand Gulf Nuclear Station (GGNS) construction site. The deficiency concerns the loss of the auto start function of the outside air fan in the Divison III l           Diesel Generator room.
As stated in AECM-82/251, we have determined that this deficiency, had it remained uncorrected, could have af fected the safety of operations of the nuclear power plant and is reportable under the provisions of 10CFR50.55(e).
It is not reportable under the provisions of 10CFR21.
The report was originally due July 25, 1982, but an extention until July 30, 1982, was granted by Mr. F. Cantrell on July 21, 1982.

: 1. Description of the Deficiency During work performed under a Maintenance Work Order (MWO) a wire on the temperature switch for an outside air fan in the Division III Diesel Generator Ventilation System was removed. As a result the fan would not automatically start upon a HPCS Diesel Generator start as described in FSAR Section 9.4.5.1.1. The loss of the auto start func-tion increases the potential of the Division III diesel generator failure and the subsequent loss of the HPCS System.
This deficiency is only applicable to Unit 1. It does not apply to either the GE or Bechtel Scope of Supply.
II. Analysis of Safety Implications This deficiency affected the Division III Emergency Diesel Generator Room Ventilation System and prevented automatic actuation of the Room Ventilation System Inlet Fan on low speed upon a Diesel Generator start signal.- The ability to start the fan manually in high speed was not affected. However the control logic was designed to automatically initiate the fan as the diesel started. The deficiency deletes auto-matic actuation capability, leaving only manual initiation control.
This could adversely affect the safe operation of the plant.
III. Corrective Actions Taken The cause of the deficiency was a personnel error. Additional work instructions, which were incomplete, were incorporated into an existing Maintenance Work Order. The authorizations required by existing main-tenance work procedures were not obtained. As a result the required retest requirements were not added.
(                 Approximately 500 Maintenance Work Orders were reviewed and no other problems were found. Therefore the deficiency affects only the Division III Emergency Diesel Generator.
A separate Maintenance Work Order was issued to properly retest the system. The retest was performed satisfactorily by 5/18/82.
All personnel responsible for this deficiency, and others who have similar responsibilities, including the individuals who reviewed and approved the Maintenance Work Order, have been notified of this prob-lem. These individuals were re-instructed on the procedural require-ments for the conduct of maintenance activities, with emphasis on the preparation and approval of work authorization changes. Furthermore, additional reviews by supervisory level personnel will be made on MWO I                 changes issued by the group prior to their releane.
