NI AGARA M0 HAWK POWER CORPORATION NINE MILE POINT UNIT 2 DOCKET NO. 50-410 A Final Report for a Problem Concerning Agastat Relay Bases Description of the Problem The problem, as described in our 30-day interim report, pertains to Agastat relay bases installed in the panels of the Power Generation Control Complex (PGCC) by the General Electric Company. During a Niagara Mohawk Quality Assurance surveillance (No. SRNMP2-101583), it was discovered that several safety related Agastat relay bases were not tightened to the metal channels in the panels to the torque specified in General Electric shop instructions. In addition, fiber washers which were supposed to be included in the installation of the Agastat relay to the metal channel in accordance with shop instructions were also omitted in several General Electric installed connections. During the surveillance, one base was discovered to be cracked.
Analysis of Safety Implications In response to the one base that was discovered to be cracked, a General Electric Field Deviation Disposition Report has been written to inspect all Agastat Relay Bases for broken studs. If one or more of the studs in the base
,       are broken, the base will be replaced.
A seismic test was performed by General Electric ir. San Jose which determined that a torque of 5 inch-pounds would adequately retain Agastat Relays in the mounting position and permit normal reliable operation. As corrective action, a Field Deviation Disposition Report will be written to perform a 100%
inspection of all Agastat Relay Bases which were not replaced in the inspection for cracked bases, to assure that the mounting screws are torqued to a minimum of 5 inch-pounds.

d A review of Gen ral Electric documentation has determined that it is acceptable to have Agastat Relay Bases installed without fiber washers between the base and the spring nut. The requirement for the installation of a fiber washer was imposed in February 1981 as a product improvement and was not backfit on those Agastat Relay Bases previously installed.
I Niagara Mohawk has not performed a detailed analysis of safety implications; however, based on engineering judgment, it is our opinion that if this problem were to have remained uncorrected, it could have adversely affected the safe operation of the plant.
Corrective Actions 1        1. A General Electric Field Deviation Disposition Report will be written to perform a 100% inspection of all Agastat Relay Bases which were not replaced in the inspection for cracked bases, to assure that the mounting screws are torqued to a minimum of 5 inch-pounds. This action will be completed by June 1, 1984.
