Containment Fan Coil Unit - Inoperable - Low Service Water Flow.
This report was initiated by Incident Reports 82-045 and 82-046.
CONDITIONS PRIOR TO OCCURRENCE:
Mode 1 - Rx Power 100% - Unit Load 1150 MWe DESCRIPTION OF OCCURRENCE:
On February 28, 1982, the Control Room Operator discovered zero service water flow indication upon starting No. 22 Containment Fan Coil Unit (CFCU). No. 22 CFCU was declared inoperable, and Action Statement 3.6.2.3.a was entered at 0612 hours.
On March 1, 1982 during the performance of Surveillance 4.0.5V-SW, the Control Room Operator noticed the service water flow to No. 23 CFCU was less than the 2500 GPM required in low speed, and the unit was declared inoperable. Because two groups of CFCU were inoperable, Action Statement 3.6.2.3.b was entered at 0540 hours.
Redundant CFCUs and both Containment Spray Systems were operable throughout the occurrence.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The cause of zero service water flow to No. 22 CFCU was Flow Control Valve 22SW223 had stuck in the closed position.
Flow Control Valve 12SW223 had stuck in the closed position, resulting in low service water flow to the CFCU. The binding of type SW223 valves is apparently associated with the accumu-lation of scale and corrosion on the valve stems, resulting from the CFCUs being idle between monthly surveillance runs.
The cause of low service water flow indication to No. 23 CFCU was the transmitter had become plugged with silt.

ANALYSIS OF OCCURRENCE:
The CFCUs operate in conjunction with the containment spray systems to remove heat and radioactive contamination from the containment atmosphere in the event of a design basis accident.
Operability of'the equipment is necessary to insure offsite radiation dose is maintained within the limits of 10CFR100.
As noted, containment cooling capability was provided by the redundant equipment; no undue risk to the health or safety of the.public was therefore involved in this occurrence. The occurrence constituted operation in a degraded mode permitted by a limiting condition for operation and is reportable in accordance with-Technical Specification 6.9.1.9b.
Technical Specification 3.6.2.3.a requires:
With one group of Containment Cooling Fans inoperable and both Containment Spray Systems operable, restore the inoperable group of cooling fans to operable status within 7 days, or be in at least hot standby within the next 6 hours and in cold shutdown within the following 30 hours.
Technical Specification 3.6.2.3.b requires:
With two groups of Containment Cooling Fans inoperable and both Containment Spray Systems operable, restore at least one of the inoperable groups of cooling fans to operable status within 72 hours, or be in at least hot standby within the next 6 hours and in cold shut-down within the following 30 hours.
CORRECTIVE ACTION:
No. 23 CFCU Flow Transmitter sensing lines were blown down to remove the silt. The transmitter tested satisfactorily and the unit was returned to service. At 1910 hours, March 1, 1982 Action Statement 3.6.2.3.b was terminated. A program to blow down transmitter sensing lines at regular intervals is in effect; the frequency of problems with silt in transmitters has been reduced.
Valve 22SW223 was exercised, service water flow was restored to l     No. 22 CFCU, and the unit was raturned to service. Action Statement 3.6.2.3.a was terminated March 5, 19 9 2 a t 0130 hours .
Due to recurrent problems of this type, daily operation of CFCUs in low speed was commenced in December 1982 (per revision of Operation Directives 40-46). No further problems have been noted since that time. Running the CFCU results in cycling of the associated control valves and has apparently eliminated the problem with the sticking of these valves.
