SCOPE This procedure describes the actions which are to be taken in the event of an illness or injury to an employee which does not involve                             '
radioactive contamination or overexposure. Injuries in which                       '
radiological considerations are involved are discussed separately in                                                  ,
the R reries of Emergency Procedures. This procedure and changes                               ..,
thereto requires PSRC review.                                                                   .
APPLICABILITY

This procedure is to be followed for incidents involving Nuclear Plant & ,
Operations personnel, or other company employees at the plant site at

the request of the Nuclear Diablo Canyon Power Plant Operating Organization. In the event of an incident involving any other. company. ,
employee (such as a General Construction Employce), perform only the ','
asterisked (*) steps in this procedure.
SYMPTOMS The symptom of an illness or injury is a person appears hurt or impaired and requires medical assistance.                                             '

INSTRUCTIONS All accidents arising out of the operation of Company owned, leased or rented vehicles, as well a9 employee owned,
* leased or rented vehicles used on Company businees, must be reported to the Supervisor in chargo immediately.
Allinjuries to persons or serious damage to property of others involving above vehicles must be repor*ed to the Safety, Health and Qaims Department Field Investigstor or,if he is unavailable, the Ger erei Office Safety, Health and Qaime Department. Such notification shall be by the festost means of communication and this report propered the same day. Answer each tguestion fully. When blank spaces are not sufficient for full statements, answer each on separate sheets and attach hereto.

2.3      In an emergency situation, the signal should sound for a minimum of one minute. A public address system announcement may follow the signal stating the classification of the emergency and any areas of the plant to avoid. The signal may be repeated.
2.4      Actuation and reset of the signal shall be as follows:

APPROVED:              Y. b PLANT MANAGER            DATE 1.0       SCOPE                                                                                 i This procedure describes the imediate emergency personnel assembly and accountability actions to be taken by all on-site personnel, security officers, contractors, and visitors in the svent of a plant emergency. This procedure and changes thereto require PSRC review.
2.0       GENERAL In the. event of an emergency situation in either Unit 1 or Unit 2, it is imperative that all personnel on-site ar1 notified of the situation, their whereabouts identified for safety and security purposes, and that they respond in a coordinated effort to the emergency.
In certain situations, (e.g., if the emergency is security related), personnel may be directed to respond in a manner other than what is stated in this procedure. In most situations, the primary notification means is the site emergency signal which l                   alerts all personnel in the vicinity of the main plant building that an emergency exists.                                               *-
2.1       The signal is produced by electronic warblers placed at               -
numerous locations throughout the plant. It has a characteristic sound; a rapid rise in pitch followed by a slower drop. The sound cycle is repeated continuously for as long as the signal remains energized.
2.2       Flashing red lights have been provided in the containment and other plant areas where the background noise level may not pemit audible perception of the electrcnic warblers.

3.0     INITIATING CONDITIONS The Shift Foreman declares that the plant is in an Alert, a Site              -.
Emergency, or a General Emergency status as defined in Emergency Procedure G-1. " Accident Classification and Emergency Plan Activation." or determines that personnel assembly and accountability is desirable, and activates the emergency signal.
4.0     IMMEDIATE ACTIONS 4.1      PGandE plant personnel engaged in critical operations or emergency recovery actions shall call their assigned assembly areas as soon as practical.
4.2       The Shift Foreman will instruct the Senior Control Operator or o'                tLperson to initiate an. all-call on group                      l calf                 sing the Health Physics Local radio. The planf-w       vo ce paging system should also be activated.
The message should indicate that the site emergency signe.1 has been sounded.
4.3       Plant security personnel shall respond as follows:
4.3.1     Those assigned to the Central Alann Station, the Secondary Alann Station, the perimeter posts, and other fixed posts shall remain at their posts and await further instructions.                                  ,
4.3.2     Those on routine patrol shall continue their patrol unless otherwise instructed by the Security Shift Supervisor.
4.3.3     All other permanent security force personnel shall call in to the Security Building for instructions.

4.5     All other personnel in the protected area not involved in                 ,
critical operations or emergency recovery shall proceed to the Security Building exit and " badge out" and then proceed to their assembly area. Personnel outside the protected area will proceed directly to their assembly area.                                -

4.6     All PGandE General Construction, construction contract personnel and their visitors will evacuate the plant buildings and proceed to their assigned Assembly Areas via their assigned badge alleys. Badges will be deposited on exit from the project.
4.7      PGandE DER, California Department of Fish and Game personnel, and their visitors, will evacuate from the plant buildings and adjacent work areas and proceed imediately to their assembly area, the PGandE Biology Lab.                          ,
4.8      DCPP visitors will be escorted to the Security Building and instructed to remain there. Accountability will be maintained by checking off those escorted visitors against the visitor sign-in log maintained at the security office.
Escorts themselves will then proceed to their own assigned assembly areas.                                                 e a              .

5.2               The Designated Assebly Area Supervisor (DAAS) for each DCPP Assembly Area will use the " BADGE-0-MATIC" computer printout, posted at each Assembly Area to check off the individuals present. The DAAS will write in the names and key card numbers of persons present but not listed on the
                                        " BADGE-0-MATIC." When the head-count is complete:

5.5     The Security Shift Supervisor shall notify the Emergency Liaison Coordinator in the TSC (or Control Room if in off-nomal hours) of the site personnel accountability, complete the "Sumary of Personnel Accountability and Assignments" log sheet (Fonn 69-10060), and forward the completed foms to the Emergency Liaison Coordinator.
5.6     If there are any unaccounted for personnel, the following actions will be initiated:                             ,
5.6.1    The Security Shift Supervisor will attempt to identify the last known location of the unaccounted persons from DAAS Reports and the Security Computer Roll Call, and provide this infomation to the Emergency Liaison Coordinator.
5.6.2    The Site Emergency Coordinator will assign appropriate personnel to perform a plant search.
If the situation dictates, the Emergency Radiological Advisor will provide a member of the Chemical and Radiation Protection Department for assistance.
5.6.3    The person leading the plant search shall contact the Emergency Liaison Coordfi.ator when contact has been made with the unaccounted for personnel.
5.7     After persons assigned to other assembly areas have assembled at the Radiological Access Control, have been              i accounted for, and have been processed through the Contamination Control Point, the DAAS will contact the               -
                            ' Emergency Liaison Coordinator and inform him of such.               I Persons assembled at Access Control may then leave the protected area via the Security Building and proceed to their designated assembly areas.
5.8     The Site Emergency Coordinator will order any necessary relocations or evacuations of assembly areas as appropriate. The DAAS of each area may relocate personnel if deemed necessary for personnel safety.

The sound cycle is repeated continuously for as long as the signal remains energized.
Flashing red lights have been provided in the Containment since the background noise level would not permit audible perception of the electronic wahblers.                                                                      l

SCOPE This procedure provides the criteria and guidance for reentry to                                                                 l affected areas of the plant site and buildings, teminating an emergency condition and initiating a program to restore the plant to its pre-emergency status. This procedure applies to emergency conditions classified as an Alert or higher emergency classification.
The procedure and changes thereto requires PSRC approval.
GENERAL The temination of an emergency and implementation of recovery efforts                                                      .
Will depend on the nature of the emergency and the status of plant systems following corrective measures. The general provisions within                                                             l this procedure are expected to be applicable to all emergencies which have an emergency classification of Alert or higher. However, it may be necessary to supplement this procedure with provisions specific to the existing plant conditions.
The overall goals of the recovery effort are to reenter the                                                                      l potentially affect areas to assess the in-plant consequences of the emergency and to perfom cleanup and repair operations to restore the plant to normal operating conditions. This effort may require additional Company resources and interfacing with outside agencies.                                                            ,
The Corporate Emergency Response Plant (CERP) describes the corporate organizational structure which would assist in these recovery efforts.
Reentry into areas potentially affected by the emergency may be desirable to determine the extent of plant damage and radiological conditions which may impede recovery efforts, and to restore safe areas to normal access provisions. Reentry should only be perfomed -
after the plant has been placed into a safe shutdown condition with TiTtTe likelihood of emergency reoccurrence.

RESPONSIBILITIES l         The Recovery Manager will make the determination to deescalate in accordanLt with the section PROCEDURE. Once the emergency 4           classifict tion has been deescalated to an Unusual Event or terminated,

ENERGENCY REENTRY TEAMS
: a. Accident Assessment The Recovery Manager will utilize all pertinent data available including area and process monitoring instrumentation, survey data from adjacent areas, observations made by evacuated personnel, recomendations by safety personnel, and any other source to detemine:
: 1)    Which plant areas are affected.
: 2)    The conditions (radiological, chemical, temperature,             3 air quality, etc.) in the areas.                                     ).
: 3)    If it is practical to reduce the potential hazards to reentry personnel (ventilating, securing systems, inc.).
: 4)     Advisable protective equipment or clothing (in addition

: 1)    The C&RP Foreman will brief the reentry team to ensure the teams have appropriate equipment and information, in accordance with the SWP.

: 2)   The emergency reentry team should approach the affected areas while frequently monitoring dose rates and other hazards anticipated.
: 3)   On the frequency agreed upon, appraise the communications contact of the progress being made by the reentry team, conditions observed, and dose rates encountered.
: 4)   Reentry team members shall check their pocket                              !
dosimeters periodically and report the results to the Team director. The frequency of these checks should be consistent with the observed dose rate.
: 5)    If at any time during the reentry, observed dose rates exceed pre-determined levels consistent with the requirements of the SWP, or if the exposure of any member of the reentry team approaches the specified maximum allowable dose for the reentry, the entire reentry team shall retreat to a safe area and report the circumstances to the director. RECOVERY

recovery operations and placing emergency regulations into effect to ensure that possibly contaminated foodstuffs are not consumed or put on the market

: 3)               Access control to potentially contaminated offsite areas will be perfonned by county emergency response personnel. Reentry into affected areas by other than emergency response personnel will be prohibited without                                  -
the approval of the county direction and control group.                               -
a)               Other recovery efforts will be coordinated between utility, county, state and federal organizations.
: b. ONSITE RECOVERY OPERATIONS
: 1)               When it has been detennined that the emergency condition has'been resolved, the following steps should be performed as applicable, and as directed by the Recovery Manager, or the Plant Operations Manager:
a)    Plant recovery activities shall be in accordance with the DCPP technical specifications and other license conditions. Specifically, during recovery operations, normal radiation exposure limits shall apply.
b)   Recovery action that plan for or may result in radioactivity releases will be evaluated by the Recovery Manager and his staff as far in advance of the release as is possible. Such releases and data pertaining to the releases will be reported to the appropriate offsite emergency response organizations and agencies in advance, even if the                                    ,
release is within nonnal technical specifications, for as long as the recovery operation continues.

EMERGENCY REENTRY AND RECOVERY c)   The recovery operation will continue until the plant is returned to its pre-emergency status, to the safest condition possible as determined by the Recovery Manager and/or Plant Operations Manager, d)   If a long-term recovery operation is indicated, modifications to the recovery organization may be established in accordance with the DCPP and Corporate Emergency Plans.
e)   If applicable, the Emergency Plan should be evaluated to ensure that an adequate emergency preparedness stature can be maintained with degraded plant conditions (i.e:               inaccessibility of evacuation assembly areas, inoperative emergency instrumentation and equipment as it relates to the EP) and appropriate corrective measures implemented as required.

RECOVERY MANAGER'S DEESCALATION CHECKLIST                -
Deescalation of an Emergency Action Level from a Site Area or General Emergency should be considered by the Recovery Manager when the following conditions exist:

[] 1)     The plant is in a safe shutdown condition or is cooling down in a safe and controlled manner.
[] 2)    All release of radioactivity to the environment greater than Technical Specification limits have been terminated.
[] 3)    The failure or damage which occurred and' increased the potential for a..
release of radioactivity has been repaired, replaced or otherwise no longer provides the potential for any additional releases.
[] 4)    The natural event (fire, flood, earthquake, hurricane, etc.) which caused the emergency has ceased or no longer possesses a danger.
[] 5)    The plant condition has been reviewed against the event classification criteria contained emergency procedure G-1 " Accident Classification and Emergency Plant Activation", and no longer requires declaration of a Site Area or General Emergency due to an unmitigated event.

An emergency condition can be considered resolved when a closeout message is issued to offsite organizations and a recovery organization established (if necessary) when the following guidelines have been met and addressed:

[] 1)             Plant conditions no longer meet the existing emergency classification criteria presented in procedure EP G-1, " ACCIDENT CLASSIFICATION AND EMERGENCY PLAN ACTIVATION" for an Alert, Site Area or General Erergency and it appears unlikely that conditions will deteriorate, requiring such an emergency declaration.
[] 2)             No additional further protective actions for the plume exposure                         ,
pathway are required and ingestion pathway considerations are being
* evaluated by th1 appropriate offsite authorities.
[] 3)             Radiation levels in affected in-plant areas are stable or decreasing to acceptable levels.
[] 4)             All releases of radioactive material to the environment greater than Technical Specification limits have ceased, and the potential for an uncontrolled release of radioactive material is diminished.
{j      [] 5)             The reactor is stable, and capable of reaching cold shutdown (Mode 5).
