UNEXPECTED REACTOR DEPRESSURIZATION AND C00LDOWN -- UPDATE This PN updates PNO-II-86-45, issued on June 27.
Preliminary information obtained by the Augmented Inspection Team now on site indicates that, when the unit's four steam generator power operated relief valves (PORVs) opened during a test, primary system temperature dropped from 550 degrees Fahrenheit to 465 degrees (rather than 450 degrees) in six minutes -- within the allowable temperature change limits.
It has also now been learned that the reactor coolant pumps were operating throughout the event, and that poor human factors design features caused the operators not to maintain ccntrol during the test.
Region II issued a confirmation-of-action letter on June 30 documenting Duke Power's commitment not to restart the reactor without NRC concurrence.
Both Duke and NRC have responded to media inquiries.
