DAMAGED MOISTURE DENSITY GAUGE AT A CONSTRUCTION SITE (STATE OF WASHINGTON LICENSEE)
On September 18,1998, Quad Cities Construction Company, a Washington State licensee contacted the Operations Center and the State of Washington to inform them of an event at a temporary job site in Pullman, Washington. The company's radiation safety officer (RSO) reported that the accident occurred when a pavement roller ran over a Troxler moisture density gauge on September 18,1998. The gauge contained an 8 millicurie cesium-137 sealed source and a 40 millicurie americium-241/berylium sealed source.
The RSO responded to the event and placed the damaged gauge into its transportation case and cordoned-off the area at the construction site. The RSO stated that the source rod had been in the shielded position when the accident occurred and that it appeared that the sources were undamaged. (The gauge electronics had been damaged.) The RSO retrieved a radiation detection survey instrument from a local hospital and conducted radiological surveys around the gauge. The survey results were reported to be normal. It was also reported that no personnel were contaminated.
The State of Washington was informed of the event by the licensee. The State has scheduled an inspection at the site for Monday, September 21,1998.
Region IV received notification of this occurrence by NRC's Operations Center at 12:30     !
