Dear Mr. Speaker:

May 30, 2025 On behalf of the U.S. Nuclear Regulatory Commission (NRC), I am forwarding the enclosed "Report to Congress on Abnormal Occurrences: Fiscal Year 2024." This submission is in accordance with section 208 of the Energy Reorganization Act of 197 4, as amended (Public Law 93-438), and the Federal Reports Elimination and Sunset Act of 1995 (Public Law 104-66),

which require the NRC to identify and report abnormal occurrences (AOs) to Congress annually.

An AO is an unscheduled incident or event that the Commission determines to be significant from the standpoint of public health or safety.

The enclosed AO report for fiscal year (FY) 2024 describes seven events involving Agreement State licensees and one event involving an NRC licensee. Seven AOs were medical events as defined in Title 10 of the Code of Federal Regulations Part 35, "Medical Use of Byproduct Material." The other AO involved two of the AO criteria: (1) a substantial breakdown of physical security, cybersecurity, or material control and accountability programs that significantly weakens protection against loss, theft, diversion, or sabotage, and (2) a serious safety-significant deficiency in management or procedural controls. No events at commercial nuclear power plants met the criteria for an AO. The NRC identified no events during FY 2024 that met the guidelines for inclusion in Appendix B, "Other Events of Interest." In Appendix C, "Updates on Previously Reported Abnormal Occurrences," the NRC included updates on AO AS23-07, "Medical Event at Baptist Health Medical Center, Little Rock, Arkansas," and AO NRC23-02, "Medical Event at Henry Ford Hospital, Detroit, Michigan."
