Contrary to the above, Detroit B31 son Procedures PCN 22.000.10,

' " Shutdown from Rated Power to Cold Shutdown,". PCM 22.000.12,"

Reactor Heatup, Cooldown, and Temperature vs. Pressure Monitoring," PCM 23.205, " Residual Heat Remval System," were not appropriate to the circumstances in that they contained no information as to how or at what temperature to maintain reactor coolant when the unit is in cold shutdown.

. B. l Contrary to the above, Detroit Edison Procedure 21.000.02,

" Conduct of Shift Operators," (sic 21.000.01, " Conduct of Shift Operations") was not appropriate as it stood mute on the point of the status of a trainee on shift and did not provide reasonable guidance as to the trainee's activities.

This is a Severity Level IV violation.

PCM 23.205, " Residual Heat Removal System," PCM 22.000.12 " Reactor Heatup, CooMown, and Temper 0ture vs. Pressure Monitoring", and PCM 22.000.10 " Shutdown fron Pated Power to Cold Shutdown" were reviewed to determine where changes were necessary to preclude this event from occurring age.in. It was determined =that P(N 23.205 was the appropriate procedure for containing steps related to maintaining the correct reactor water temperature. Therefore, PCN 23.205 was revised to include steps to establish ard maingain a Reacgor Pressure Vessel (RPV) water temperature band of 140 F to 1757, including:

methods for recording RPV temperature and pressure methods for establishing the initial cooldown rate methods for increasing and decreasing the cooldown rate guidance to maintain RPV water gemperatuge as constant as possible within the band of 1407 to 175 F.

Although procedural inadequacy was the root cause of this event, good operating practice dictates that responsibility for a job function being performed by a. trainee rest with the licensed S

. irdividual responsible for that job furction. Therefore,

'.e disciplinary action was administered to the trainee ard to the operator responsible for supervising the trainee..

s-Procedure 21.000.01 has been revised to clearly state that "the C,

responsible operator is always responsible and shall be held 1j3 accountable for the activities assigned to his job function

-.whether they_ are specifically performed by him or his trainees".

It has been further revised to provide guidance concerning trainees' activities on shift. Additionally, urgent required reading was issued concerning this procedure change and the lessons' learned from this event.

The changes made to PCM 23.205 were made to prevent future problems with maintaining proper cold shutdown conditions.

B. 'The changes to POM 21.000.01 were made to prevent future problems with control of trainees on shift.
