Statement of Violation Criterion XVI in 10CFR50, Appendix B, states that for significant l

l conditions adverse to quality measures shall assure that the cause of the condition is determined and corrective action taken to preclude repetition.

l Contrary to the above, corrective action taken in response to l

safety-related, Jamesbury butterfly valve failures has been inadequate l

to prevent repeated failures from the same, common cause. These failures occurred in September 1984 (T2300-F409), Novenber 1985 (T4600-F401), and August 1986 (T2300-F410).

q This is a Severity Level IV violation.

l Corrective Actions Taken and Results Achieved The three Jamesbury butterfly valves identified above.were each repaired following their individual failures. Valve T2300-F409, the first of the three valves to fail, was reassembled using a new carbon steel shaft and wafer assenbly. Valves T4600-F401 and T2300-F410 were reassembled using stainless steel shafts and wafers in place of their original carbon steel components.

Corrective Stens that will be Taken to Prevent Recurrence The remaining carbon steel shafts and wafers in safety-related Jamesbury valves will be replaced with stainless steel shafts and wafers. Twenty-three of the thirty-three safety-related Jamesbury l

butterfly valves already have stainless steel components. The l

remaining ten valves will be modified by replacing their carbon steel i

i shafts and wafers with corresponding stainless steel components prior l

to startup following the first refueling outage. These valves are:

.i modified as described above.. The twenty-three safety-related Jamesbury valves with stainless steel internals will continue to be monitored by stroke-time testing in accordance with IST Program.
