The information is as initially received without verification or evaluation, and is basically all that is known by Region IV staff in Arlington, Texas on this date.

RECEIPT OF CONTAMINATED PACKAGES The licensee's customer, St. Cloud Hospital, St. Cloud, Minnesota, returned empty syringe holders housed within two " ammo" shipping containers to the Syncor radiopharmacy in St. Paul, Minnesota. Originally the containers housed diagnostic technetium 99m unit doses prepared by Syncor. While conducting a routine package receipt survey, Syncor personnel detected 305,000 disintegrations per minute (dpm) per 100 square centimeters on the exterior of one " ammo" container, and 2290 dpm per 100 square centimeters on the exterior of the second returned " ammo" container. Both contaminates were determined to be technetium 99m. Syncor notified St. Cloud Hospital (an NRC licensee) and conducted surveys of their delivery vehicle and driver. No additional contamination was detected. Other customer hospitals visited by the driver were also notified of this event by Syncor and did not report any contamination problems.

Region IV received notification of this occurrence by the Headquarters Operations Center at 6 p.m. (CDT) on September 15,1997.

Region IV has informed Region lll and NMSS. Region til has informed the state of -

Minnesota and has also initiated discussions with St. Cloud Hospital.

This information has been discussed with the licensee and is current as of 10 a.m. (CDT),

September 16,1997.
