Response to 85-31-01 " Inadequate Test Procedure" Reason For The Violation Preoperational type A test procedure 1-PT-57-1 was issued and being used to conduct the Integrated Icak Rate Test, even though six differential pres-sure instrument valves and one motor operated valve which were required to be properly positioned were in fact not included in the procedure's valve lineup.

Corrective Steps hhich IIave Been Taken and Results Achieved

'Ihe valves were identified during the performance of the test. Maior Change Request (MCR) No. 4 was issued to revise 1-PT-57-1 to add the misn-ing valves to the lineup.

Corrective Steps khich Will Be Taken to Avoid Further Violations

'Ihe valve lineup now delineated in 1-PT-57-1 will be re-verified.

All changes made to 1-PT-57-1 will be evaluated for incorporation into the surveillance test procedure (STP) .

Date hhen Full Carpliance Will Be Achieved The valve lineup delineated in 1-PT-57-1 will be re-verified by Septanber 16, 1985.

Incorporation of changes into the STP as discussed above will be canpleted by Decanber 31, 1986, prior to the scheduled perfornance of the STP.
