030-20273/85-01 Tulsa, Oklahoma License No.: 17926-02 During an NRC inspection conducted on July 8,1985, violations of NRC requirements were identified.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violations are listed below:
A.
License Condition 12 states that licensed material shall be used by, or under the supervision of Walter.P. Haney, M.D.
Contrary to the above, licensed material was used by an individual other than Walter P. Haney, M.D. during the period March 13, 1984, to July 8, 1985. This individual is an authorized user under another'NRC license.
This is a Severity Level IV Violation (Supplement VI)'.
B.
10 CFR 20.401 requires, in part, that the licensee maintain the radiation exposures of all individuals for whom personnel monitoring is required under 10 CFR 20.202.
Contrary to the above, the licensee did not maintain records showing the radiation exposures of nuclear medicine personnel for the period of January 5,1985, to May 4,1985.
This is a Severity Level V Violation (Supplement VI).
Pursuant to the provisions of 10 CFR 2.201, AMISUB, Inc., is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
