GPPR R8/129 During an NRC inspection conducted August 5 - September 27, 1985, one
-
violation of NRC requirements was identified..The violation involved failure to provide traceable material test records.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985),the violation is listed below:
Failure to Provide Traceable Material Test Records 10 CFR Part 50, Appendix B, Criterion VIII states that measures shall be established for the identification and control of materials, parts, and components, including partially fabricated assemblies.
These measures shall assure that identification of the item is maintained by heat number, part number, serial number, or other appropriate means, either on the item or on records traceable to the item, as required throughout fabrication, erection, installation, and use of the item.
Revision II of the licensee's STP " Quality Assurance Program Description" (QAPD), Section 8.0, " Identification and Control of Haterials, Parts and Components," requires the establishment of requirements to ensure compliance with 10 CFR 50, Appendix 8.
Contrary to the above, it was determined that material test reports for eight (8) components used in the fabrication of the 6 X 6 fuel racks (Units 1 and 2), could not be retrieved.
This is a severity level IV violation (10CFR Part 2 Supplement IE)

Pursuant to the provisions of 10 CFR 2.201, Houston Lighting & Power Company is hereby. required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including:
(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
