Dennett and C. C. IIarbuck during the period June 3-7, 1985, of activities authorized by NPC Construction Permit CPPR-145 for River Bend Station Unit No.1.
Gulf States Utilities Cmpany's (GSU) response to the Notice of Viola-tion 85-42-01, " Control of Tmporary Alterations," is provided in the enclosed attachment. This empletes GSU's response to the Notice of Viola-tion.

Response to 85-42-01 " Control of Tmporary Alterations" Corrective Steps Khich Itave Been Taken and Results Achievuxi h responsible Startup and Test Engineer (SrE) landed the leads inmrxliate-ly after the NPC inspector contacted the Startup and Test (SU&T) depart-ment. The responsible STE was asked whether he knew he was in violation of 17 when he lifted the leads and his reply was Test Instruction (TI) affirmative. The reason he gave was that he was busy conducting the test and was going to log the liftcd Icads at a later date. The responsible STE was terminated frcm River Bord Station.
On June 5, 1985 a change was approved to TI-17 Section 3.1.2.
The change rcroved a contingency that a S1E cculd place a tcnporary alteration into effect without documentation in the tmporary alteration Icg f f it did not rmain in place for core than one (1) work shift. A clarification mcrcoran-dum was distributed on June 6,1985 to all SU&T personnel notifying the of the change and providing the following guidance:
"To clarify, all tmporary alterations must be documentcd in the tm[n-rary alteration log unless a person is continuously present at the intnediate location of the alteration. The "one work shift" provision has been deletal. If you have lifted a lead or installed a jumper and have to leave the work area for a rest break, lunch or other reason, then you must either (a) have another person present who is cogizant of the alteration rm.ain at the work area, (b) remove the alteration, or (c) document the alteration in the Tmporary Alteration Log. There shall be no deviations fram thin critoria."
Training was held prior to the incident for Startup & Test Pornonnel on May 29, 1985 en TI-17. It is GSU's opinion that the responsible STE knowingly violated TI-17 and that re-training on TI-17 is not rerplired for all per-sonnel.
