'Ihis letter is in response to the Notice of Violation contained in NRC I&E Inspection Report No. 50-458/85-33. The inspection was perfomed by Messrs. L. D. Gilbert and W. M. McNeill during the period April 15 - June 27, 1985, of activities aut orized by NRC Construction Permit CPPR-145 for River Bend Station Unit No. 1.

Response to 85-33-01 Inspectic.. Reports Reason for the Violation

This violation resulted frcm the failure of the Inspection Report (IR) reviewer to obtain concurrence of the originating inspector for a change which affected inspection results. Alternately, the reviewer could have either performed and documented a verificaticn inspection or justified the change on the IR, but he did not do so.
Correction of inspection documentation which does not affect inspection results is normally accmplished by the reviewer without further approvals.
Corrective Steps Which Have Been Taken and Results Achieved Type "C" IR P-5700083 was issued to cause all inspection disciplines to review a sample of their IR's for similar occurrences. Of the 1462 IR's reviewed by the other disciplines, no instance was found where results were changed by the reviewer without documented justification.
For the As-Built discipline where the original finding was observed, 100%
of the IR's were reviewed (approximately 1000) . This review identified five additional IR's which were changed by the reviewer and could not be justified. Reinspection of the affected supports for the changed attribute listed on these five IR's revealed four to be acceptable and one unaccept-able. Unsatisfactory IR W5200677 was issued for this support.
Corrective Steps hhich Will Be Taken To Avoid Further Violations Documented retraining has been given to all SWEC inspection pensonnel authorized to perform review of Inspection Documentation. Instruction was given on the proper methods to be amployed for corrections and/or changes to Inspection Documentation.
