This is an update to a previous report submitted on September 13, 1982. The following paragraphs describe the event.

Vessel fill, after sensor installation work, commenced at 1000 hours0.0116 days <br />0.278 hours <br />0.00165 weeks <br />3.805e-4 months <br /> on August 13, 1982, using CRD pumps. This was signed off in Temp Directive 04-1-01-B21 Temp 1 step 5 after clearing Brown Tag clearance 3446. Level-was to be raised to normal level (30" to 40"). The decision to start clearing red tag 4428 prior to returning the RPV level transmitters to service was directed by management through operations in order to expedite the closing of the temporary directive in preparations for CRD friction testing. The ECCS pump breakers were to remain racked out until the RPV icvel instrumentation was completed. These actions were implemented without appropriate changes and reviews of the temporary directive. At 1415, red tag 4428 was cleared and ECCS breakers and valves were returned to service. At this time RPV level was being monitored on B21 LT N691B. Level was read at 1340 hours0.0155 days <br />0.372 hours <br />0.00222 weeks <br />5.0987e-4 months <br /> at -50 inches using this instrument.

At 1845 the shift superintendent requested I & C to begin valving in RPV level instrumentation per step 6 of the temp. directive. .This action led to the ECCS initiation at 2104 on August 13, 1982.

Corrective action initiated in the previous report has been completed. A permanent procedure for filling and venting vessel level instruments and returning them to service in an orderly and controlled manner has been written. The procedure, 07-S-13-23, was approved and issued November 17, 1982. No future action is necessary. This is submitted as a final report.
