No. 23 Auxiliary Feedwater Pump - Inoperable - Failed Governor.

This report was initiated by Incident Report 82-053.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1160 MWe DESCRIPTION OF OCCURRENCE:

On March 13, 1982, during performance of Surveillance Procedure SP (O) 4. 0. 5.p, No. 23 Auxiliary Feedwater Pump would not attain the specified minimum speed _ of 3500 RPM. At 1131 hours0.0131 days <br />0.314 hours <br />0.00187 weeks <br />4.303455e-4 months <br /> the pump was declared inoperable, and Action Statement 3.7.1.2.a was entered.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The auxiliary feedwater pump failed to reach speed because vibration had caused the governor linkage to be out of adjust-ment and there was leakage past the overspeed ball check valve.

ANALYSIS OF OCCURRENCE:

The operability of the auxiliary feedwater system ensures that g the Reactor Coolant System can be cooled down to less than 350 F from normal operating conditions in the event of a total loss of offsite power.

Technical Specification 3.7.1.2.a requires:

With one auxiliary feedwater pump inoperable, restore the refluired auxiliary feedwater pump to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, or be in at least het standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in hot shutdown within the following 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />.

Inoperability of No. 23 Auxiliary Feedwater Pump, therefore, constitutes operation in a degraded mode permitted by a limiting condition for operation and is reportable in accordance with Technical Specification 6.9.1.9.b.

Inspection Order Cards have been issued to prevent recurrence of the problem with the governor linkage and a similar problem. -

with the trip linkage documented in LER 81'-08 9 /0 3L . These cards require an annual tightness and visual inspection of the linkages on both Nos. 13 and 23-Auxiliary Feedwater Pumps. N' FAILURE DATA: ,

2 A similar occurrence was ex'perienced on No. 23 Auxiliary Feedwater Pump and was documented in LER 81-089/03L. In this occurrence, vibration had caused the trip mechanism bolts to loosen.  :
