TOLEDO EDISON COMPANY DAVIS-BESSE UNIT ONE NUCLEAR POWER STATION SUPPLEMENTAL INFORMATION FOR LER NP-33-78-125 DATE OF EVENT: October 31, 1978 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Inoperable Diesel Generator 1-2 Ventilation Inlet Dampe r Conditions Prior to Occurrence: The unit was in Mode 2, with Power (MRI) = 83 and Load (MWE) = 0.

Description of Occurrence: During the conduct'of ST 5081.01, " Diesel Generator Monthly Test", on Diesel Generator Unit 1-2, the room temperature rose to 1100F, The ventilation fans were on and the damper motor was running but damper HV 5336A would not open.

Diesel Generator 1-2 was declared inoperable at 2045 hours0.0237 days <br />0.568 hours <br />0.00338 weeks <br />7.781225e-4 months <br /> on October 31, 1978.

This placed the unit in the Action Statement of Technical Specification 3.8.1.1, which requires both Diesel Generators to be operable while the unit is in Modes 1, 2, 3 or 4.

'I During the investigation of this failure, it was determined the preventative main-tenance work order 78-2298 had determined the damper would not open on October 26, 1978, but at this time, it was not realized the damper was required for emergency diesel generator long teon operation. A work request was generated by the person-nel performing the preventative maintenance.

b Designation of Apparent Cause of Occurrence: Under Maintenance Work Order 78-1426-42, damper HV 5336A was inspected, and it was discovered the linkage was out of adjustment and bound in a guide.

Analysis of Occurrence: There was no danger to the health and safety of the public l

or to unit personnel. Diesel Generator 1-1 was operable and would have been availa-ble if offsite power was lost. Diesel Generator 1-2 would start and would have e

continued to function for an undetermined amount of time.

Corrective Action: The linkage was disassembled and readjusted. The damper was stroked six times to verify proper operation. Surveillance Test ST 5081.01 was completed and Diesel Generator 1-2 declared operable at 2345 hours0.0271 days <br />0.651 hours <br />0.00388 weeks <br />8.922725e-4 months <br /> on October 31, 1978. This removed the unit from the Action Statement of Technical Specification

4 TOLEDO EDISON COMPA!N DAVIS-BESSE UNIT ONE NUCLEAR POWER STATION SUPPLEMENTAL INFORMATION FOR LER NP-33-78-125 PAGE 2 Maintenance Work Order 78-2643 was issued to inspect the linkage on inlet damper HV 5329A to Diesel Generator Room 1-1 Ventilation Fan. There has been a preven-tative maintenance schedule to inspect the operation of the dampers on a bi-monthly basis. At this time, it is felt that this frequency is adequate.

A copy of this report will be routed to all Shif t Foremen and Maintenance supervisory personnel so it is realized these dampers are required for emergency diesel generator operability. .

Failure Data: There have been no previously reportable failures of the Diesel Generator Room Ventilation Dampers.
