Equipment malfunction. The operate / test switch was placed in test'during 4

SI performance and could not be returned to operate.

Analv:.is of ()c"urrence There was no danger to the health or safety of the public, no release of activity, no damage to the plant or equipment. and no resulting significant chain of events.

- Corrective Action:

The vents and fans were isolated and tagged. The continuous air monitor was repaired and returned to service at 1530.on 7/22/81. This is considered a' random event and no recurrence control is required.
