LLR NITACliMENT - RO # 1-81-55 Facility: BSEP Unit No.1 Event Date: 07/06/81 This event ocrarred as a result of incomplete . maintenance, where following-repairs to the rod position indication system associated with rod 22-07, the rod probe to instrument cable connector was not sufficient ly tightened. This caused a loss of position indication at rod positions "44", '45", "48", and " full out" indication on the RTGB and the plant computer printout, ie- designed, the cable connector utilizes a clamp which prevents a visual check of the connector integrity after installation of the clamp. During the investigation of this event, it was revealed that the previous maintenance which led to this event had been performed without an LCO. As a result, an operability check was not performed to ensure the rod had been properly repaired. The failure to initiate j-~'

.an LCO on the problem worked during the maintenance which led to this event will be reported in LER 1-81-48. As a result of this event, all I&C maintenance personnel will review this report and the corrective actions required as a result of the failure to initiate an LCO will be outlined in LER 1-81-48.
