t Apparent Cause of Occurrence:

A ' leak at the threaded connection between the fluid reservcir and the snubber, probably caused by improper thread sealant.

Analysis of Occurrence:

There was no danger to the health or safety of the public, no release of and no resulting significant activity, no damage to the plant or equipment, chain of events.

Corrective Action

Snubber will be rebuilt and a suitable titread connection sealant used.

Inspection intervals for snubbers will be changed per SI 4.6.H because this snubber was found inoperable.
