IDENTIFICATION OF' OCCURRENCE: Replacement of Diesel Generator (DG) 1-1 turbo--

, <: charger due'to anticipated failure. .

Conditions Prior to Occurrence: The unit was in Mode 1, with' Power (HWT) = 2060, and Load-(MWE) = 730.

Description'of Occurrence: On February 8, 1978, while performing Surveillance Test ST. 5081.01, " Diesel-Cenerator Monthly Test", Operations personnel noticed Ethat Diesel Cenerator (DC) 1-1 was making unusual noises which appeared to be originating from an air start motor. On further investigation by Maintenance personnel, fit vas. determined that'the noise was coming from the turbocharger, j

The turbocharger puts more-air'into the diesel cylinders allowing better effi-ciency and higher horsepower. On a loss of the turbocharger, the diesel would

- .not be able to reach full ~1oad power requirements. It was suspected the turbo-

< charger reliability was inadequate and the. Shift Foreman declared Diesel Genera-tor 1-1' inoperable at 1245, hours. This.placed the unit in Action Statement "A"

of Technical . Specification 3.8.1.1, which requires
two independent Diesel Cencra- ,

tors in' Mode's 1,J2, 3 and 4. Operations personnel test started Diesel Generator 1-2 Eand performed : Surveillance Test ST 5080.01, "Of fsite AC Sources" as required by the Action Statement throughout the occurrence.

Designation'of Apparent Cause of Occurrence: The turbocharger was sent back to-

.the vendor, Power. Systems, where-an investigation and analysis were made. It

.showed .that the fprimary cause of the' malfunction was a roller clutch failure. The al failure.was due to an overload condition of the clutch which could have been due to loading during acceleration and/or running at a no load rated speed for long

- , periods of time.

-Analysis of Occurrence: .There was no danger to the health and safety of the public '

.or to tation personnel. Diesel Generator 1-2 was operable if a loss'of normal power su'ppliesihad occurred.

' Corrective? Action: 'The vendor, Power Systems, was contacted and it was decided to replace .the . turbocharger.: A service representative was 'sent to the site.
