During the performance of MRI 303 it was found that the EECW flow was below the minimum allowed flow on 3B RHR pump seal heat exchanger.

Conditions ' Prior to Occurrence: ,

Unit 1 @ 95% steady state power.

Unic 2 0 95% steady state power.

' Unit 3 @ 76% steady state power..

Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. Describe. _

Redundant systems were proven operable per SI 4.5.B;1.c, 4.5.B.1.b, 4.5.'A.l.c (loop I & II) 4.5.A.l.' (loop I & II), 4.5. A.1 Supp A (all Unit 3 d/g's).

Apparent Cause of Occurrence: .

Shells and mud restricted th'e flow at the EECW inlet to the RHR pump seal heat exchanger.

Analysis of Occurren e:

There was no danger to the health or safety of the public, no damage to plant j or equipment and no release of activity.

, ' Piping was disassembled and :lened.
