Apparent Cause of Occurrence:

Vacuum pump failure.

Analysis of Occurrence:

There was no danger to the health or safety of the public, no damage to the plant equipment, and no resulting significant chain of events.

The vacuum pump was' replaced. Personnel were cautioned to be more attentive to
