50-302/80-015/03L-1 Fecility: Crystal River Unit #3 Rtport Date: 19 May 1980 Occurrence Date: 6 March 1980 Identification of Occurrence:

Both Reactor Coolant loops inoperable, contrary to Technical Specification 3.4.1.

Conditions Prior to Occurrence:

Mode 5 cold shutdown.

Description of Occurr'ance; At 1827 while performing an inspection of the Non-nuclear Instrumentrcion System, Decay Heat suction isolation valve DHV-3 began to close inadvertently. The operating Decay Heat pump DHP-1A was stopped immediately. DHV-3 was returned to its full open position immediately. Decay Heat System flow was returned to normal at 1835.

Designation of Apparent Cause:

Initial investigation and troubleshooting efforts revealed no apparent cause for this event. Subsequent investigation also failed to identify the cause. Performance Testing Pro-cedure PT-124, Decay Heat Valve 3 Operational Check, was conducted on 10 May 1980 to determine the operability of DHV-3 and to address appropriate corrective action. The results of the test were satisfactory. No hardware problems were dis-covered.

Analysis of Occurrence:

No affect upon the plant or general public.

Corrective Action:

Since the apparent cause could not be determined, corrective action to preclude recurrence cannot be addressed. A review of Engineered Safeguards Functional Test SP-130, used to con-duct the initial inspection revealed a weakness in assuring proper test switch position for DHV-3. As a result, a revision to SP-130 is being implemented to alleviate this concern.
