Tennessee Valley Authority Sequoyah Nuclear Plant LER SUPPLEMENTAL INFORMATION SQRO-50-327/8065 Technical Specification Involved 3/4.7.12 Reported Under Technical Specification Sections 3.7.12 and 6.9.2 Date of Occurrence: 4/30/80 Time of Occurrence: 1530 hours0.0177 days
0.425 hours
0.00253 weeks
5.82165e-4 months
Unit 1 Identification and Description of Occurrences

,. , ,,5 ;

Four (4) mechanical penetrations passing through a fire zone boundary leading into the emergency gas filter treatment system filter room in the auxiliary building were discovered to be breached. This room contains. equipment which

    • is safety-related for unit 1.

' Condition Prior to Occurrence Unit 1 in Mode 5 _

Action specified in the Technical Specification met due to inoperable equipment.

Describe. ,

Within one (1) hour following discovery of the condition it was verified that the fire detection system was functional on one side of the fire barrier and hourly fire watch coverage was verified as required by LCO Action Statement 3.7.12. These requirements shall be met until repairs are completed and the penetration fire barriers verified as restored to functional condition as required by SR 4.7.12.b.

Apparent Cause of Occurrence These penetrations are associated with reactor building No. 2 and were evidently a* omitted by Construction as being a requirement for unit I due to their location.

.t Analysis of Occurrence mn .

There was no hazard to public health or safety.

~

Corrective Action c All four (4) mechanical penetration will be repaired and verified as functional as required by SR 4.7.12.b by June 13, 1980
