Rupture dice failed on RCIC exhaust causing trip of RCIC. Failure occurred during SI with itnit at 34% power.

,lr I.arent Cause of Occurrence:

Teflon sheet failed providing no protection to rupture disc under vacuum condition.

Cause of the Teflon sheet failure is unknown but is believed to be due to normal fatigue.

fqiplysis of Occurrence:

There was no significant resulting event and no danger to the health or safety of the public.

Corrective Action

The rupture disc was replaced. Approval has been received to change to an improved type rupture disc.
