Ice loading and high winds resultad in a " galloping" phenomena whereby the phases were either touching or becoming close enough to flash over. This created a fault condition requiring operation of line protective relaying.

Cen31itions Prior to Occurrence:

Unit 1 @ 0, refueling outage Unit 2 @ 0, cold shutdown for maintenance Unit 3 @ 100%, normal operation Acthn specified in the Technical Specificition Surveillance Requirements met due to inoperableyfu_ipment. Describe.

None. Regulatory Guide 1.93 allows continued operation at full or reduced power for a limited time, if it is safer, than to effect an immediate shutdown on the loss

,of some of the required power sources.

Ann.irent Cause of Occurrence: The Athens 161-kV breaker opened and locked out from pedtective zone relays turvere permitted to operate on a fault condition on the Trinity line because pilot wire carrier blocking of the relay scheme was not in service for the Browns Ferry station. The Athens line remained de-energized until a substation operator arrived and reclosed the line breaker. Carrier blocking pilot signals were g g g i g gegu g og r y s g g g ion switching and clearance procedures did Amlysis of Occurrence:

There was no hazard to the public health or safety.

Corrective Acticn: sh'utdown loads were supplied from unit i until offaite power was restored. Contingency procedures were developed to supply station loads from the 500-kV system. System switching and clearanca procedures will be revised,at the dispatching of fice level to include the carrier pilot relaying signal switches in the return to service of Browns Ferry lines. Man will be stationed at Athens substation during adverse weather conditions.
