Facility: Crystal River Urit #3 Report Date: 4 March 1980 occurrence Date: 27 February 1980 Identification of Occurrence:

Failure to maintain the sampling frequency of the Action Statement referenced in Technical Specification 3.4.8. This created an e. vent reportable under Technical Specification 6.9.1.8.b.

Conditions Prior to Occurrence:

Mode 3 (hot standby)

Description of Occurrence:

On February 26, 1980 following a plant transient, which resulted in a class "B" emergency, the RCS dose equivalent I-131 Itsitation was exceeded. The four (4) hour sampling frequency was initiated at 0058 on 27 February 1980 as per the Action Statement require =ents of the Technical Specifica-tion. At 0830 on 27 February 1980 it was discovered that the RCS sampling frequency for dose Equivalent 4-131 had not been maintained. Investigation revealed that an RCS sample due at 0458 was not taken until 0614 Upon discovery of this event the correct sampling Laterval was resumed.

Designation of Apparent Cause:

As a result of additional non-routine sampling requirements during a plant transient, a dose Equivalent I-131 sample was inadvertently overlooked.

Analysis of Occurrence:

No effect upon the plant or general public.

Corrective Action:

Personnel were instructed on the 5ecessity.of thoroughly - ._

reviewing data to identify Standard Technical Specification Action items. . _ .
